ClinConnect Summary

This clinical trial is focused on finding ways to reduce high blood pressure, also known as hypertension, in the South Side of Chicago, an area that faces significant health challenges. Hypertension affects many adults in the U.S. and can lead to serious health problems like heart disease and stroke. The study aims to adapt successful health strategies from a program used in California to fit the needs of local communities, particularly through churches and community health workers.

To participate in this trial, you must be an adult aged 18 to 89 living in the area of the participating clinics or churches. If you join, you can expect to receive support for managing your blood pressure through community-based activities, which may include regular health check-ups and education on healthy living. The research team will work closely with local community members to make sure the program meets their needs and is effective in improving health outcomes. This study is actively recruiting participants and aims to share its findings to help other communities in the future.
