ClinConnect Summary

The Improving Diabetes Equity and Advancing Care Study is a clinical trial aimed at finding the best way to help people with diabetes manage their condition more effectively. This study will look at different combinations of support options, like diabetes self-management training and remote glucose monitoring, to see which methods work best together to lower blood sugar levels, measured by a test called A1c. Participants will be randomly assigned to start with one type of training or monitoring and then may switch to another type after six months based on their progress.

To join this study, you need to be at least 18 years old and have either type 1 or type 2 diabetes. The trial is specifically looking for participants who identify as African American/Black or Latinx and have a blood sugar level (A1c) above 7%. You'll also need to have a smartphone that works with the Glooko app and be a current patient at the study center. If you decide to participate, you can expect to receive support and training tailored to your needs to help you manage your diabetes better. This study is currently recruiting participants, so if you meet the criteria and are interested, you could have the opportunity to contribute to important research that aims to improve diabetes care in your community.

Eligibility criteria

  • Inclusion Criteria:
  • A diagnosis of type 1 or type 2 diabetes
  • 18+ years of age
  • Able to provide consent
  • Identify as African American/Black or Latinx
  • most recent HbA1c \> 7%,
  • Have a smartphone compatible with the Glooko application
  • Have completed at least one Center appointment (i.e. current patient)
  • Completed at least 3 months of their assigned study condition
  • Exclusion Criteria:
  • Already an active user of RGM
  • Actively working with a CHW
  • Diagnosis of gestational diabetes (without type 1 or type 2)
  • Has a Power of Attorney
  • Be suffering from Stage-V renal disease or undergoing dialysis.
  • Be suffering from a severe form of cardiovascular disease (NYHA Class III and IV)
