This study is testing whether a “produce prescription” program helps people with Type 2 diabetes who also have low income or food insecurity. The idea is to give a financial incentive to buy fruits and vegetables, and to see if this improves blood sugar control (measured by HbA1c) and also affects things like diet, weight, blood pressure, food security, diabetes-related worry, and overall quality of life. The researchers will also look at how feasible and cost-effective running these programs is. The study is being done at five U.S. sites and compares people who receive the produce prescription to those who get usual diabetes care, over about 4–6 months, with measurements at the start and around month 6.
Who can participate? Adults 18 and older with Type 2 diabetes who are food insecure or eligible for Medicaid/SNAP and who are patients at a participating site. Exclusions include pregnancy or plans to move away during the study. About 600 people are planned to enroll. This is a health-services research study, not a drug or device trial, and results are not yet available. Sites include Chicago, Fort Wayne (Indiana), New Orleans (Louisiana), Marlton (New Jersey), and Santa Fe (New Mexico).

Eligibility criteria
- Inclusion Criteria:
- • 1. diagnosed with Type 2 diabetes
- • 2. have clinically screened positive for food insecurity and/or be income-eligible to receive Medicaid and/or SNAP-benefits
- • 3. ≥ 18 years of age, and
- • 4. be a patient at a participating healthcare site.
- Exclusion Criteria:
- • 1. people who pregnant or nursing and
- • 2. plans to move away during the study period.
