This clinical trial, called I CAN DO Surgical ACP, is designed to help people aged 65 and older who are planning to have elective surgery. The study encourages these patients to think about and share their wishes for medical care in case they become unable to make decisions during or after surgery. This process is called Advanced Care Planning (ACP), and it helps ensure that patients’ preferences are known and respected.
If you are 65 or older and have been referred for surgery, you might be eligible to join the study, as long as you haven’t already completed an ACP recently (within the last 3 to 5 years) or participated in this study before. Participants will be guided through the ACP process before surgery, which can provide peace of mind by making sure your healthcare team and loved ones understand your wishes. The study is currently recruiting patients, and it welcomes people of all genders.

- Inclusion Criteria:
- • - Older Adults (age 65+) referred for surgical evaluation
- Exclusion Criteria:
- • 1. ACP on file within 3 years prior to surgery (UCSF and UCI) and 5 years (UMN)
- • 2. Was previously randomized into intervention for a prior surgery during the 18-month study period
