This clinical trial, titled "Survivorship After HNC," is looking at how well patients with head and neck cancer (HNC) follow their care plans after treatment. The study aims to see if providing a personalized care plan and counseling session can help improve the way doctors meet the needs of HNC survivors. Previous smaller studies showed that patients who received this type of support had better outcomes. Now, researchers want to test this approach with 304 patients who have been treated for HNC. Participants will be divided into two groups: one will receive usual care, and the other will receive the personalized care plan along with a counseling session to discuss their needs and concerns.
To be eligible for the trial, participants must be at least 18 years old, speak and understand English, and have been diagnosed with certain stages of head and neck cancer. They should also have completed their main treatment within the last 3 to 6 months. Those with other active cancers, advanced disease, or significant cognitive issues will not be included. If you join, you can expect to receive either standard care or a more tailored support plan aimed at helping you navigate life after cancer treatment. The researchers will also follow up with you after a year to see how you're doing and how satisfied you are with your care.

Eligibility criteria
- • Patient Participants
- Inclusion Criteria:
- • Age 18 years or older
- • Willing to provide informed consent
- • Diagnosis of stage I-IVA head and neck mucosal cancer (larynx, hypopharynx, oral cavity, oropharynx, nasopharynx)
- • Last definitive treatment (surgery, chemotherapy, radiotherapy) between 3 and 6 months prior to enrollment
- • English speaking, reading and writing
- Exclusion Criteria:
- • Second concurrent non-cutaneous malignancy
- • Metastatic disease
- • Clinically apparent cognitive impairment
- • Suspected residual disease after treatment completion
