This clinical trial is studying how effective a medication called tranexamic acid is in reducing blood loss during a major surgery called total laryngectomy, which involves removing the voice box. The researchers want to compare two ways of giving this medication: one directly through an IV (intravenous) and the other by applying it directly to the surgical area (topical). The goal is to see if the direct application can control bleeding just as well as the IV method, but with potentially fewer risks.
To participate in this study, you should be between 18 and 70 years old and planning to have a total laryngectomy. It's important that you are generally healthy and don’t have conditions that affect blood clotting or a history of blood clots or allergic reactions to tranexamic acid. If you join, you can expect to receive one of the two types of tranexamic acid during your surgery and be monitored for any changes in blood loss. This trial is currently recruiting participants, and your involvement could help improve surgical care for others in the future.

Eligibility criteria
- Inclusion criteria:
- • - Patients of either sex, aged between 18-70 years old, ASA physical status I, II undergoing total laryngectomy
- Exclusion Criteria:
- • Patients with coagulopathy, a history of thromboembolism or a history of tranexamic acid allergy or complication will be excluded.
