This clinical trial is studying a new type of swallowing training called instrument-assisted early progressive resistance swallowing training. The purpose is to see if this method can help improve the muscle strength needed for swallowing in patients who are critically ill and have had a tracheotomy. Researchers want to find out if this training is more effective than simply practicing swallowing on its own and to understand any potential problems patients might encounter while participating.

To be eligible for this trial, participants need to be at least 18 years old, have undergone a tracheotomy after being on a ventilator, and be receiving treatment in an Intensive Care Unit (ICU) with an expected hospital stay of more than two weeks. Participants will engage in daily swallowing training for two weeks and will be tested on their swallowing muscle strength once a week. This trial aims to not only improve patients' swallowing abilities but also ensure their safety during the training process.

Eligibility criteria

  • Inclusion Criteria:
  • Aged ≥ 18 years old
  • Tracheotomy after intubation and ventilator ventilation
  • Receiving treatment in ICU, and expected hospital stay \> 2 weeks after pneumonectomy
  • Be able to cooperate to complete the maximum anterior tongue pressure test
  • Exclusion Criteria:
  • Inability to cooperate with tongue pressure testing and swallowing training, including disturbance of consciousness, oropharyngeal neuromuscular or motor impairment
  • Patients refuse to participate in this study
  • Significant malocclusion and facial asymmetry
