This clinical trial is studying whether adding a medication called dexamethasone to a local anesthetic called bupivacaine can improve visibility during a specific type of ear surgery known as tympanomastoid surgery. Good visibility is important for surgeons to perform the procedure safely and effectively. The trial is currently not recruiting participants, but it aims to include adults between the ages of 21 and 70 who are classified as ASA I or II, meaning they are generally healthy or have mild health issues. Participants should be scheduled for surgery that lasts less than five hours.

If you or a loved one is considering participating, you should know that not everyone will be eligible. For example, people who have allergies to local anesthetics or dexamethasone, serious blood disorders, or infections at the injection site cannot take part. Those who are uncooperative or refuse to participate will also be excluded. If eligible, participants can expect to receive the study medication during their surgery, and the study will help researchers understand how this combination affects surgical outcomes.

Eligibility criteria

  • Inclusion Criteria:
  • 1. ASA I \& ASA II patients
  • 2. Patients undergoing tympanomastoid surgery.
  • 3. Age group: from 21 to 70 years old.
  • 4. Duration of surgery less than 5 hours.
  • Exclusion Criteria:
  • Patient refusal.
  • Uncooperative patients.
  • Allergy to local anesthetics or dexamethasone.
  • Anatomical abnormalities include malformations, deformities, growths, or structural irregularities.
  • Infection at injection site.
  • Coagulopathy: PTT \> 40 seconds, INR \> 1.4, platelet count \< 100x10⁹. or drug induced bleeding disorders
