This clinical trial is looking at how self-management education and follow-up phone calls can help people with Chronic Obstructive Pulmonary Disease (COPD) manage their symptoms better. COPD is a serious lung condition that causes breathing problems and can worsen suddenly, known as an exacerbation. The study will involve patients who have been hospitalized for a COPD exacerbation, and it will compare three different groups: one group will receive both education and follow-up calls, the second group will only get education, and the third group will receive standard care without additional support. The researchers want to see if the education and follow-up help improve patients' ability to take care of themselves and manage their symptoms.
To participate in this study, individuals need to be diagnosed with COPD for at least a year and have a severe form of the disease (stages III or IV). They must be at least 18 years old, have been hospitalized for at least three days due to COPD, and have a mobile phone they can use. During the study, participants will be asked questions to track their progress at the beginning, and again at one, three, and six months later. This trial is currently recruiting participants, and it aims to provide valuable insights into how education and support can help improve the lives of those living with COPD.

Eligibility criteria
- Inclusion Criteria:
- • Diagnosed with COPD for the last 1 year
- • Diagnosis of Global Initiative for Chronic Obstructive Lung Disease (GOLD) stage III or IV COPD documented by pulmonary function testing
- • Patients hospitalized for acute exacerbation of COPD;
- • Hospitalized for at least 3 days
- • Having a personal mobile phone and the ability to use it,
- • Being enough conscious and oriented to answer the questions,
- • Voluntary patients
- Exclusion Criteria:
- • Under 18 years old
- • Withdrawal from the study at any phase
- • Inability to be contacted by phone/mobile phone,
- • Patient death
