This clinical trial is looking to find a better way to identify patients in hospitals who might have chronic health conditions, like diabetes or heart disease, without them even knowing it. Many people are unaware they have these conditions, which can lead to serious health issues if not treated early. The trial aims to create a digital tool that uses existing patient information, like blood test results and medical history, to help doctors pinpoint those who may be at risk and need further testing.
To participate in this trial, you should be at least 18 years old, currently admitted to a participating NHS hospital, and registered with a primary care doctor. If you join, your hospital records will be used to help test a new system that could improve how doctors detect chronic diseases. It's important to note that if you've chosen not to share your data for research, you won't be able to take part. The goal of this study is to make sure patients receive the right care sooner, ultimately helping to prevent serious health complications.

- • Admitted to a participating NHS hospital
- • Registered with a primary care practice
- Exclusion Criteria:
- • Has "opted-out" of having their data used for research purposes using the national data opt-out service
