The E3 Hypertension clinical trial is looking to improve the way we manage high blood pressure, also known as hypertension. This study will compare a new approach that combines medical care with support for social needs to the usual care patients receive in primary care clinics. The goal is to see which method helps people better control their high blood pressure.
To participate in this trial, you need to be an adult aged 18 or older, identify as African American and/or Latinx, and have uncontrolled stage 2 hypertension, meaning your blood pressure is consistently 140/90 or higher. You also need to be a patient at one of the eligible Rush primary care clinics, have a smartphone, and an email account. If you join the study, you can expect to receive personalized support to help manage your blood pressure, along with regular check-ins. It's important to know that some people, like those who have had an organ transplant or are on dialysis, cannot participate. This trial aims to make a real difference in how hypertension is managed, especially for communities that may face additional challenges.

Eligibility criteria
- Inclusion Criteria:
- • Adults 18 years or older
- • African American and/or Latinx
- • Uncontrolled stage 2 hypertension, BP \>/= 140/90
- • Patient is following with Rush primary care provider in eligible Rush primary care clinics
- • Patient has a smart phone (Android or iOS)
- • Patient has an email
- Exclusion Criteria:
- • Organ transplant recipient
- • On dialysis
- • Patient is already participating in another remote hypertension monitoring program
- • Patient is not interested in participating in the program
- • Patient has already participated in the E3 Hypertension program or the E3 Diabetes program
