This clinical trial is studying two different ways to numb a lower back tooth when it has a painful condition called symptomatic irreversible pulpitis, which means the tooth’s nerve is inflamed and causing pain. The trial compares a common numbing method called the inferior alveolar nerve block (IANB) with a different technique called intraligamentary injection, which delivers anesthesia directly around the tooth’s ligament. The goal is to find out which method works better to control pain during root canal treatment.
Adults aged 18 and older who have this painful tooth condition in a lower molar and who respond to a simple cold test may be eligible to join. People with allergies to the numbing medicine, pregnant or breastfeeding women, or those who have taken painkillers recently are not eligible. If you participate, you can expect to receive one of the two numbing techniques during your dental treatment, and your pain levels will be carefully recorded to see which method provides better relief. This study is currently recruiting participants and aims to help dentists improve pain control during root canal procedures.

Eligibility criteria
- Inclusion Criteria:
- • Adults aged 18 years or older
- • Presenting with symptomatic irreversible pulpitis in a mandibular molar
- • Vital response to cold testing
- • Ability to understand and provide informed consent
- • Available for follow-up and data collection
- Exclusion Criteria:
- • Known allergy to lidocaine or epinephrine
- • Pregnancy or breastfeeding
- • Use of analgesics or anti-inflammatory drugs in the previous 6 hours
- • Non-vital or previously treated teeth
- • Significant systemic disease (ASA III or above)
- • History of psychiatric or neurological disorders affecting pain perception
