This clinical trial is looking at how a mobile health system can help people with chronic and complex health conditions, like Spinal Cord Injuries, Cerebral Palsy, and Traumatic Brain Injuries, manage their health and improve their overall wellness. Participants using this mobile health system will be compared to those who just receive standard care to see which approach is more beneficial for their self-management skills.
To be eligible for this study, individuals must be at least 12 years old and diagnosed with one of the specified conditions. They should also live in a community setting, meaning they are not in a care facility, and be able to use a smartphone at a basic level. Participants will be guided on how to use the mobile health system and will be assessed to ensure they can benefit from it. This trial is currently recruiting, and both males and females can participate. It's a great opportunity for those looking to enhance their health management skills with the help of technology.

Eligibility criteria
- Inclusion Criteria:
- • 1. 12 years or older.
- • 2. have a diagnosis of a chronic and complex condition such as Spina Bifida, Spinal Cord Injury, Cerebral Palsy and Traumatic Brain Injury, etc.
- • 3. live in a community setting (not within a residential facility that provides care to them).
- • 4. pass all the functional screening tests which include basic usage of a smart phone, and impairment severity assessments
- Exclusion Criteria:
- • 1. Diagnosis of severe intellectual disability or severe and persistent psychiatric illness.
- • 2. Failing all the screening tests due to severe impairments that cannot be accommodated by the mHealth system (for patient participants)
