This clinical trial is studying whether using non-invasive ventilation at home can help patients with Chronic Obstructive Pulmonary Disease (COPD) avoid future severe breathing problems after they have recovered from a serious episode of respiratory failure. Non-invasive ventilation is a method that helps patients breathe more easily without needing to have a tube inserted into their throat. The goal of the study is to see if this home treatment can reduce the chances of experiencing another serious breathing crisis for those who have already been treated for one.
To be eligible for this trial, participants must be COPD patients who have survived a serious breathing episode and have been off ventilation support for at least five days. They should have stable blood gas levels indicating they can breathe independently but still have elevated carbon dioxide levels. The study is open to all adults aged 65 to 74. However, those with severe sleep apnea, other causes of breathing failure, serious health conditions, or significant psychological issues will not be included. Participants can expect to receive non-invasive ventilation equipment to use at home and will be monitored to see how well this treatment works in preventing future health issues.

Eligibility criteria
- Inclusion Criteria:
- • COPD patients who survived after an acute episode of hypercapnic respiratory failure
- • patients weaned from ventilation ( non invasive ventilation or mechanical ventilation) prescribed for acute episode since at least five days with following arterial blood gas : pH \> 7.35 and PCO2 \> = 45 mmHg
- Exclusion Criteria:
- • severe obstructive sleep apnea ( DI \> 30/h)
- • Non COPD cause of respiratory failure
- • Serious comorbidity
- • Adverse psychological status
