This is an observational study in routine medical practice that looks at adults at risk for type 2 diabetes and for cardiovascular (heart and blood vessel) problems. Researchers will use tests to see how the body handles sugar and insulin (an oral glucose tolerance test) and will check the health of the neck arteries (carotid arteries) to measure things like artery thickness and plaque. The goal is to find signs that predict who will develop type 2 diabetes or vascular disease and to guide personalized prevention, using standard lifestyle advice and medicines that doctors already use in usual care (not a new drug trial).
Who can join? The study is open to adults aged 18 to 100 who are being treated in private practice in Southern California and who meet criteria showing they are at risk for type 2 diabetes. People already taking medications specifically for diabetes are not eligible. The plan is to enroll about 5,000 participants and follow them for up to about 20 years—initially looking back at past records for up to 10 years, then following people prospectively for 10 more years. Participants will have periodic medical evaluations, glucose testing, and vascular assessments roughly every 2 years to see if and when diabetes develops and to learn which factors affect risk. This is an observational study, not a drug trial, and results aren’t available yet.

- • American Diabetic Association (ADA)/American Association of Clinical Endocrinologists (AACE) criteria for patients at risk for developing type 2 diabetes
- Exclusion Criteria:
- • Patients with prior treatment with medications for type 2 diabetes
