This clinical trial is studying whether a type of brain imaging called computed tomography perfusion (CTP) can effectively identify patients with acute ischemic stroke (AIS) who might benefit from a treatment called reperfusion therapy. This therapy is designed to restore blood flow to the brain and reduce disability when administered within a certain time frame after a stroke. The researchers want to see if CTP can work just as well as another imaging method, magnetic resonance perfusion (MRP), in finding patients who will respond positively to the treatment.

To be eligible for this study, you need to be between 18 and 80 years old and provide consent to participate. You should have had a stroke more than 4.5 hours ago, but still meet certain imaging criteria related to the extent of brain damage. Participants will undergo both CTP or MRP imaging and, if eligible, may receive reperfusion therapy. This study is important because it could help improve treatment options for stroke patients and potentially increase the number of people who receive timely and effective care.

Eligibility criteria

  • Criteria:
  • Inclusion Criteria:
  • 1. Provision of informed consent;
  • 2. Male and female adults aged 18-80 years old;
  • 3. For patients accepting CTP or MRP over 4.5 hours after stroke onset, imaging criteria: infarct core volume \<70mL, perfusion leison volume / infarct core volume \>1.2, and absolute mismatch \>10 mL.
  • Exclusion Criteria:
  • 1. Standard contraindications to rt-PA;
  • 2. Contraindication to imaging with contrast agents;
  • 3. Pre-stroke mRS score of ≥2 (indicating previous disability);
  • 4. Participation in any investigational study in the previous 30 days;
  • 5. Any terminal illness such that patient would not be expected to survive more than one-year.
