This clinical trial is studying how the diaphragm, which is the main muscle used for breathing, changes in structure and function in patients who are on mechanical ventilation or life support while waiting for a lung transplant. Researchers want to understand how these changes can affect patients' health and recovery. The trial aims to gather information from both patients who are receiving respiratory support and those undergoing different lung surgeries.
To be eligible for this study, you must be receiving help with breathing as you wait for a lung transplant and be officially listed for the transplant. There are also two control groups: one for patients having lung transplant surgery for specific lung diseases and another for patients undergoing elective chest surgery without any past lung issues. Participants in the study can expect to have their diaphragm examined and monitored over time, helping doctors learn more about the effects of mechanical ventilation on this important muscle. It's important to note that people with certain conditions, like neuromuscular disorders or specific chest deformities, cannot participate in this trial.

Eligibility criteria
- Inclusion Criteria (Cases):
- • Receiving a respiratory support modality as a bridge to lung transplantation
- • Formally listed for transplantation
- Inclusion Criteria (Control Group 1):
- • - Undergoing lung transplantation for either obstructive or restrictive lung disease
- Inclusion Criteria (Control Group 2):
- • - Undergoing elective thoracic surgery without any prior history of chronic pulmonary parenchymal disease
- Exclusion Criteria (Cases and Controls):
- • Previously diagnosed with neuromuscular disorder
- • Received invasive mechanical ventilation for \>48 hours in the preceding four weeks
- • Thoracic cage deformity
