This clinical trial is looking at a new way to manage Type 2 diabetes in China by using a shared care model, which has been successful in Taiwan. The goal is to improve diabetes management by having healthcare professionals—like diabetes experts, educators, and community doctors—work together with patients. They will use an interactive online platform and mobile apps to help patients monitor their blood sugar levels and manage their diabetes more effectively. This new approach will be tested in six diabetes centers, comparing the results with patients who are receiving the usual care over five years.
To be eligible for this trial, participants need to be adults with Type 2 diabetes. However, those who have not signed the consent form or who have severe heart, liver, or kidney problems cannot take part. If you join, you can expect to receive close support and monitoring from a team of healthcare providers, which may help you reach better health goals related to your diabetes. This study is currently recruiting participants, and it aims to find out if this new method can lead to better outcomes for diabetes management compared to the traditional approach.
