The DC-eALERT trial is studying a new way to help prevent serious blood clots, known as venous thromboembolism (VTE), in patients who are hospitalized for medical illnesses. These patients are at higher risk for VTE, especially after they leave the hospital. The trial will test a computerized alert system that reminds doctors to prescribe a longer course of medication to prevent these clots for patients who are at risk and not currently receiving any preventive treatment at discharge. By doing this, the study aims to see if more patients will receive this important medication, which may help reduce the number of people who experience symptomatic VTE after leaving the hospital.
To be eligible for this trial, participants must be at least 40 years old, hospitalized for conditions like heart failure or infections, and have reduced mobility. They should not be receiving any blood clot prevention treatment when they leave the hospital and must have additional risk factors, such as being over 60 years old, having a history of blood clots, or having cancer. If you join the study, you can expect to be part of a group that will help test this alert system, potentially making a difference in how hospitals prevent blood clots in patients like you or your loved ones in the future.

Eligibility criteria
- Inclusion Criteria:
- -≥40 years of age, are hospitalized for acute medical illness (heart failure, respiratory failure, infectious disease, rheumatic disease, or ischemic stroke), have reduced mobility, are not prescribed thromboprophylaxis at hospital discharge, and have one additional risk factor for VTE:
- • Age ≥60
- • Prior VTE OR
- • History of cancer
- Exclusion Criteria:
- • - Prescribed thromboprophylaxis at hospital discharge
