This clinical trial, called Frailty Rehabilitation, is studying how a special rehabilitation program can help older adults improve their physical abilities and overall health. Frailty is a condition where a person has multiple health issues that can make them weaker and more likely to fall or get sick, especially in older adults. The researchers want to see if a 4-month frailty rehabilitation program can help people aged 65 and older who are living with frailty and sarcopenia (which means muscle weakness) by improving their physical function compared to those who only do regular exercise.
To participate, individuals need to be at least 65 years old, able to walk independently for 25 meters, and at high risk for mobility challenges. They should also be fully vaccinated against COVID-19 and have medical clearance to join the program. Participants can expect to attend sessions at the YMCA up to twice a week, where they will engage in activities designed to boost their strength and function. This study is important because it aims to create a new model for managing frailty in Canada and could help many older adults live healthier, more active lives.

Eligibility criteria
- Inclusion Criteria:
- • Community-dwelling ≥65 years of age
- • Able to independently ambulate 25m with or without walking aid
- • At high risk for mobility disability/functional limitations
- • Received medical clearance
- • Can arrange transportation to the YMCA up to 2x/week
- • Proof of being fully vaccinated against COVID-19 and proof of identification
- Exclusion Criteria:
- • Unable to speak or understand English
- • Currently attending a group exercise program
- • Currently in a drug optimization study/program
- • Currently taking protein supplements daily
- • Significant cognitive impairment where they may have difficulty following two-step commands in group exercise
- • Receiving palliative/end of life care
- • Unstable angina or heart failure
- • Unable to attend for more than 20% of trial duration
