This clinical trial is studying a new approach to total knee replacement surgery, called a fast-track protocol. This method aims to help patients recover more quickly after surgery by using better planning and treatments based on research. The trial will compare this fast-track approach with standard care to see which one results in better recovery, shorter hospital stays, fewer blood transfusions, and less need for pain medication.
To be eligible for this study, participants should be adults aged 65 and older who have knee osteoarthritis and are planning to have a total knee replacement. They need to have a body mass index (BMI) below 32 and be in good overall health before surgery. Participants can expect to receive either the fast-track or standard care during their treatment, and they will be monitored closely for their recovery progress. It's important to know that people with certain health conditions or who need crutches before surgery may not qualify for this trial.

Eligibility criteria
- Inclusion Criteria:
- • patients affected by knee osteoarthritis, eligible for primary total knee arthroplasty
- • BMI \< 32
- • Time up ang go test \</= 12 seconds
- • American Society of Anesthesiologists physical status classification system (ASA) \</= 2
- • preoperative hemoglobin (HB) \>13 g/dl
- • patients eligible for spinal anesthesia
- • presence of a care-giver
- Exclusion Criteria:
- • psychiatric diseases
- • preoperative use of crutches
- • ASA \> 3
- • preoperative HB \< 13 g/dl
