ClinConnect Summary
This clinical trial is focused on helping patients who have been hospitalized for serious heart conditions, specifically acute heart failure or a heart attack. Many patients struggle to understand health information, which can lead to mistakes with their medications and a higher chance of going back to the hospital soon after they leave. The study aims to create a simple education program tailored for those who find it hard to understand medical instructions. This program will provide clear information about their medications and care, helping them follow their treatment plans better after they return home.
To participate in this trial, you need to be at least 18 years old and have been hospitalized for a second acute heart issue. You should also be living at home before your hospitalization and have health insurance. Participants can expect to receive support that is specifically designed to meet their needs, even if they have a short hospital stay. The goal is to reduce medication errors and improve overall health for individuals with low health literacy. This innovative approach could eventually be used for many other chronic health conditions, making it easier for patients to manage their care effectively.
