This clinical trial is comparing three different techniques for treating periodontitis, a serious gum disease that can lead to tooth loss. The study will look at how well these techniques work in helping to regenerate the gums and improve overall dental health. Participants will be assigned to either a method that uses marginal access (a way to reach the gums from the top) or apical access (accessing from the bottom).
To join the trial, patients need to be diagnosed with periodontitis and have certain conditions, like specific depths of gum pockets and certain types of bone defects in their jaw. The trial is currently recruiting participants aged 65 to 74, and both men and women can apply. If eligible, participants can expect to receive one of the treatment methods being studied and will be monitored for their progress. It's important to know that individuals with certain health issues or specific dental problems may not be able to participate.

Eligibility criteria
- Inclusion Criteria:
- • patients diagnosed with periodontitis.
- • plaque index and bleeding index of \< 30%.
- • periodontal lesions with pocket probing depth \> 5 mm.
- • intrabony defect \> 3 mm.
- • intrabony defect configuration including a 1 and/or 2-wall component, always involving the buccal wall.
- Exclusion Criteria:
- • patients with systemic diseases that contraindicated treatment.
- • third molars.
- • teeth with incorrect endodontic or restorative treatment.
