This clinical trial is looking at how a comprehensive education program can help Veterans with chronic kidney disease (CKD) make better choices about their treatment options as they approach end-stage renal disease (ESRD). The goal is to improve their understanding of CKD and the different types of dialysis available, especially home dialysis, which is a safe and effective option that many Veterans are not currently using. By enhancing their knowledge and confidence, the researchers hope to see more Veterans choosing home dialysis, leading to better health outcomes and potentially lower healthcare costs.
To participate in this study, Veterans must be 18 years or older, registered for healthcare from the NF/SG VHS, and have advanced CKD (stage 4 or 5) but not yet on dialysis. Unfortunately, some individuals may not be eligible, including those who do not speak English, are homeless, live in assisted facilities, have dementia, or have a life expectancy of less than six months. Participants can expect to engage in educational sessions that will help them make informed decisions about their care. If successful, this trial could provide a valuable approach to improving kidney care for Veterans.

Eligibility criteria
- Inclusion Criteria:
- • Veterans registered to receive healthcare from the NF/SG VHS
- • 18 years or older
- • Advanced CKD (stage 4 or 5) and not on dialysis
- Exclusion Criteria:
- • Non-English speakers
- • Veterans who are homeless or living in assisted living facilities or nursing homes
- • Veterans with diagnosed dementia
- • Veterans with the life-expectancy less than 6 months
- • Any additional special concerns of a Veteran's provider
