This clinical trial is studying a new way to help older adults stay active during their hospital stay to prevent them from losing their physical abilities. When elderly patients are hospitalized, they often spend a lot of time in bed, which can lead to a decline in their strength and mobility. This study aims to implement a special program that encourages physical activity, both while they are in the hospital and after they go home. The goal is to see how effective this program is and whether it can help reduce the need for additional healthcare services after they leave the hospital.
To participate in this trial, individuals must be aged 65 or older, have a planned discharge to their home, and be able to speak and understand either French or English. There are no specific exclusion criteria, but doctors will assess if participants can safely engage in physical activity. Those involved in the trial can expect to receive personalized exercise plans designed to fit their needs, helping to keep them active during their recovery. Overall, this study aims to improve the health and independence of older adults after hospitalization.

Eligibility criteria
- There is no eligibility criteria but the doctor will have to consider these criteria :
- • be aged 65 and over,
- • planned discharge to home
- • Presence of self-criticism
- • No PA contraindication
- • And ability to speak and understand French and/or English
- Exclusion Criteria:
