This clinical trial is looking at how individual peer support can help stroke patients when they return home after their hospital stay. After a stroke, patients and their families face many challenges as they adjust to new health needs and caregiving roles. The study aims to see if having someone who has experienced a stroke themselves—called a peer-helper—can provide better emotional and social support to patients during this transition, compared to the usual support they currently receive. Researchers believe that this personalized support might improve the patients' quality of life and make them feel more empowered as they recover.

To be eligible for this study, participants must be adults who have had their first confirmed stroke and are planning to return home from a rehabilitation center. They should have a certain level of independence (measured by a score) and be able to give written consent. The trial is open to all genders, and patients living in the Rhône department can participate. Those who are unable to understand surveys about quality of life, or who have unstable medical or psychological issues that could affect their participation, will not be included. Participants can expect to receive individual support from a peer-helper, which may help them feel more confident and supported as they adjust to life after a stroke.

Eligibility criteria

  • Inclusion Criteria:
  • Adult patient,
  • Having had a first confirmed, ischemic or hemorrhagic stroke
  • Managed in the participating rehabilitation center
  • Whose discharge to home directly from the rehabilitation center is planned
  • Presenting a modified Rankin score of 1 to 3 when deciding to leave the stroke center
  • Having given its written consent
  • Whose main residence is located in the Rhône department
  • Aphasic patients may be included if a caregiver can follow up with the case manager
  • Exclusion Criteria:
  • Patient living in an institution prior to stroke
  • included in a gerontological network before stroke
  • Patient unable to understand quality of life questionnaires
  • Patient with unstable medical or psychological conditions who, in the opinion of the investigator, could lead the subject to be non-compliant or uncooperative during the study or could compromise patient safety or participation in the study
  • Patient under guardianship or curatorship
  • Patient not affiliated to a social health insurance
