To be eligible for this study, participants must be adults aged 65 and older who have been seen at the Yale New Haven Health System and have a diagnosis of heart failure. Unfortunately, individuals under 18 years old cannot participate. If you or a loved one joins the trial, you’ll be contributing to important research that could improve heart failure care for many patients. The study is currently recruiting participants and aims to provide valuable insights into how heart failure is monitored and treated in real-time.
