This clinical trial is investigating a new approach to diagnosing prostate cancer using MRI scans. Currently, many men are advised to undergo a prostate biopsy after an MRI, even if no cancer is detected. This trial aims to see if there's a better way by having the decision to biopsy based on a discussion between the doctor and the patient about their individual cancer risk. The study will enroll 600 men, comparing two groups: one that receives a biopsy after the MRI regardless of the results, and another that only undergoes a biopsy if both the doctor and patient agree it’s necessary based on the risk assessment.
To be eligible for this trial, participants must be 18 years or older, speak Finnish, and have a suspicion of prostate cancer based on their PSA levels or a doctor’s examination. They should also be able to understand the study and give informed consent. Participants can expect to have their prostate health monitored and to take part in discussions about their care, which is an important part of this trial. This study is currently recruiting participants and aims to improve the way prostate cancer is diagnosed, potentially reducing unnecessary procedures for men with a low risk of cancer.

Eligibility criteria
- Inclusion Criteria:
- • Age: 18 years or older
- • Language spoken: Finnish
- • Clinical suspicion of prostate cancer, based on: serum level of PSA from 2,5 ng/ml to 20 ng/ml and/or abnormal digital rectal examination according to the referral physician
- • Mental status: Patients must be able to understand the meaning of the study
- • Informed consent: The patient must sign the appropriate Ethics Committee (EC) approved informed consent documents in the presence of the designated staff
- Exclusion Criteria:
- • previous diagnosis of prostate cancer
- • any contraindications for MRI
- • any other conditions that might compromise patient's safety, based on the clinical judgment of the responsible urologist
- • bilateral hip prosthesis
