This clinical trial is looking at how kidney transplant patients can be monitored at home instead of having to visit the hospital for all their follow-up appointments after surgery. After the first couple of weeks post-transplant, participants will be randomly chosen to either continue with standard care at the hospital or to manage some of their check-ups from home. Those in the home monitoring group will take a small blood sample from a finger prick, send it to the lab, and then receive a phone call from their transplant doctor the same day to discuss the results. The goal is to see if this home-based monitoring works just as well as traditional hospital visits.
To be eligible for this trial, participants must be adults over 18 who have received a kidney transplant, are on specific medications to prevent rejection, and are considered to have a standard level of immunological risk. However, pregnant women and those currently experiencing rejection episodes cannot participate. The trial is currently recruiting patients at the Oslo University Hospital-Rikshospitalet transplant center, and it aims to provide more flexible and convenient care for kidney transplant recipients while ensuring that they continue to receive the necessary medical attention.

Eligibility criteria
- Inclusion Criteria:
- • Kidney only transplant
- • Immunosuppressive therapy With at least one of; tacrolimus, cyclosporine, everolimus, sirolimus, mycophenolate
- • Standard immunological risk; no DSA, no PRA, not ABO-incompatible transplant
- • Age above 18 years
- • Followed at Oslo University Hospital-Rikshospitalet transplant center
- • Written informed consent
- Exclusion Criteria:
- • Pregnant or breast feeding female recipients
- • Ongoing acute rejection episodes at time of inclusion
