This clinical trial is looking at how to improve care for older adults who have finished chemotherapy for cancer. The researchers want to see if adding a special visit to assess overall health and well-being, called a geriatric assessment, can help older patients feel better and more satisfied with their care compared to just receiving a survivorship care plan alone. A survivorship care plan is a guide that helps patients manage their health after cancer treatment.
To participate, you need to be at least 65 years old, have been diagnosed with cancer, and completed chemotherapy with the goal of curing the cancer. However, if you have advanced dementia or live in a nursing home, you won’t be eligible. If you join the study, you can expect to receive either the survivorship care plan alone or the care plan along with the health assessment visit. This trial is currently recruiting participants, and your involvement could help improve care for older adults in the future.

Eligibility criteria
- Inclusion Criteria:
- • 1. age 65 years and above and
- • 2. Have diagnosis of cancer and have completed chemotherapy with curative (not palliative) intent and
- • 3. have been scheduled to receive a survivorship care plan from an oncology navigator.
- Exclusion Criteria:
- • 1. advanced dementia or serious cognitive impairment that would impede ability to participate in the interview
- • 2. nursing home resident.
