The IHOPe project is a clinical trial aimed at helping older adults, specifically those who are 75 years and older and considered frail, manage their health better. This study focuses on creating a digital support system that allows frail older people to work closely with healthcare professionals, family members, and others in their community to identify their health needs and available resources. Participants will engage in personalized phone calls with healthcare providers and use a user-friendly digital platform designed to include those who might feel left out of technology. The goal is to enhance their ability to take charge of their health, reduce hospital visits, and ensure they feel like equal partners in their healthcare journey.
To be eligible for this trial, participants must be at least 75 years old, live in the Västra Götalands region, and have been identified as frail by a healthcare provider, but they should not currently be in the hospital. Those needing end-of-life care, without a registered home address, participating in other conflicting studies, or with severe cognitive issues will not be included. The trial is currently recruiting participants and aims to improve the health and well-being of older adults while making healthcare more accessible and supportive for them.

Eligibility criteria
- Inclusion Criteria:
- • National registration within the Västra Götalands region (VGR), community dwelling people (men and women) aged 75 and older living in ordinary housing, and screened as frail at a primary care center or the emergency department but not hospitalized.
- Exclusion Criteria:
- • In need of palliative care in the final stages of life, no registered address, participating in any other conflicting randomised study, or cognitive dysfunctional (not oriented to time, place and person).
