This clinical trial is studying how blood pressure (BP) and arterial stiffness change throughout the day and night in patients who are at higher risk for cardiovascular disease (CVD), like those with kidney problems or certain inflammatory conditions. Normally, our blood pressure drops at night, which helps protect our hearts. The researchers want to learn more about these patterns in patients who might not experience this natural drop and see how they compare to healthy individuals. By understanding these patterns, the goal is to find better ways to reduce the risk of heart-related issues in these patients.

To participate in the study, you need to be between the ages of 18 and 90 and have a condition that puts you at increased risk for CVD, such as chronic kidney disease (CKD), acute kidney injury (AKI), or having received or donated a kidney. Healthy volunteers from the community will also be included as a comparison group. If you join the study, you can expect to have your blood pressure and arterial stiffness measured over 24 hours, along with blood and urine tests during both day and night. This important research could help develop more effective strategies to improve heart health for people at risk.

Eligibility criteria

  • Inclusion Criteria:
  • * Patients will be eligible to take part in the study if they attend NHS Lothian inpatient or outpatient services and can be classified as being at increased risk of CVD. This will include, but is not limited to, the following subgroups:
  • 1. CKD as defined by the Kidney Disease Outcome Quality Initiative (K/DOQI) classification
  • 2. AKI as defined by the Kidney Disease Improving Global Outcomes (KDIGO) classification
  • 3. Small vessel vasculitis
  • 4. Kidney transplant recipient
  • 5. Kidney donor We will also recruit a healthy control group from the community.
  • Exclusion Criteria:
  • 1. Age \<18 years and \>90 years
  • 2. Lack of ability to provide informed consent
