The Stroke-Card Registry trial is studying how to improve care for patients who have experienced an ischemic stroke or a transient ischemic attack (TIA), which are both types of strokes caused by blood flow issues in the brain. This research focuses on a special program called STROKE-CARD care, designed to help manage health after a stroke. The goal is to understand how well this program works in helping patients recover and to collect valuable information for future studies on stroke treatment and prevention.
To participate in this trial, individuals must be at least 18 years old and have had an acute ischemic stroke or a high-risk TIA within the last 30 days. Participants will need to provide written consent and live within the area of the participating medical centers. Patients will be followed over time to see how their care and recovery progress, which is important to improve long-term outcomes for stroke survivors. This study is currently recruiting participants, and all genders are welcome to join.

Eligibility criteria
- Inclusion Criteria:
- • Patients with acute ischemic stroke or high-risk TIA (ABCD2-Score ≥ 4 points) treated at the Department of Neurology of each study center within 30 days after onset
- • Age ≥ 18 years
- • Written informed consent
- Exclusion Criteria:
- • Patients living outside the catchment area of the participating centers
- • Patients under law enforcement or within mandatory military service
- • Age \< 18 years
