Functional and Cognitive Rehabilitation of Hoarding Disorder

This clinical trial is studying a new approach to help Veterans with Hoarding Disorder (HD), a condition that can significantly affect their daily lives and overall well-being. The researchers aim to personalize a treatment called Cognitive Rehabilitation and Exposure/Sorting Therapy (CREST) to make it more effective for each individual. In this study, 130 adult Veterans with HD will be randomly assigned to receive either the personalized treatment or standard case management. The goal is to see how this tailored approach can improve their quality of life and reduce the severity of their hoarding symptoms over 24 sessions.
To participate, Veterans must be 18 years or older and have a confirmed diagnosis of HD. They should also be stable on their medications for at least six weeks. Participants will receive care in the comfort of their own homes through telemedicine and home visits, making it easier to engage in the treatment. Throughout the study, the researchers will monitor various outcomes, including the participants' quality of life and their progress in recovery. This trial is an important step in understanding how to better support Veterans with Hoarding Disorder, ultimately helping them lead more fulfilling lives.

Eligibility criteria
- Inclusion Criteria:
- • Veterans age 18 and older
- • voluntary informed consent for participation
- • DSM-5 diagnosis of HD as measured by the Structured Interview for Hoarding Disorder
- • HD as a primary diagnosis, and 5) stable on medications for at least 6 weeks
- Exclusion Criteria:
- • current psychosis or mania as measured by the Mini-International Neuropsychiatric Interview
- • current or history of any neurodegenerative disease
- • concurrent participation in any form of exposure-based psychotherapy
- • suicide ideation will be monitored by the clinicians during sessions and VA standard suicide measure will be completed at all assessment points
