This clinical trial is studying a new way to help people with Hepatitis C (HCV) who have recently been released from jail or prison. The goal is to create a support program where peers—people who have gone through similar experiences—will help these individuals connect with healthcare providers to start and continue their treatment for HCV. The researchers want to understand what challenges people face in getting started with treatment and completing it, so they can improve care for those who need it.
To participate in this trial, you must be at least 18 years old, currently incarcerated or recently released (within the last six months), and diagnosed with chronic Hepatitis C. You also need to live in the Upstate area of South Carolina and be able to communicate in English or Spanish. If you join, you can expect to receive support from peers who understand your situation, as well as help in navigating your HCV treatment. The study is currently looking for participants, and your feedback could help improve healthcare for others in similar circumstances.

Eligibility criteria
- Inclusion Criteria:
- • currently incarcerated or recently released from a U.S. jail or prison (6 months)
- • Chronic HCV with documented detectable viral load
- • 18 years old
- • Fluent in English or Spanish
- • Resident of the Upstate area of South Carolina
- Exclusion Criteria:
- • Unable to sign informed consent
- • Life expectancy of less than 1 year
- • Plans to relocate from the Upstate area of South Carolina in the next 6 months
