ClinConnect Summary
The REmote moBile Outpatient monitoring in Transplant 2.0 trial is looking to see if a new mobile health program can help improve communication between patients and their healthcare providers. This program aims to reduce the need for unexpected hospital visits and trips to the emergency room for adult patients who have received solid organ transplants, such as heart, liver, or kidney transplants.
To participate in this study, you must be an adult who has had a heart, liver, or kidney transplant and be able to use a smartphone in English. Unfortunately, if you have difficulty understanding health information or can't follow instructions from the program, you won't be eligible. If you join the study, you'll use a special app called Reboot, which is designed to help you stay in touch with your healthcare team. This trial is not yet open for enrollment, but it aims to make managing your health after a transplant easier and more effective.
