This clinical trial is looking at how virtual reality (VR) can be used during total knee replacement surgery (TKA) for older patients. The main goal is to see if using VR can help reduce the amount of sedatives patients need while still keeping them comfortable and satisfied during the surgery. Participants will be compared to those who listen to music or receive standard care to see which method works best.
To be eligible for this trial, patients need to be 60 years or older and scheduled for a primary total knee replacement using spinal or regional anesthesia. Those with certain medical conditions, such as active infections or a history of seizures, or who are planning to use general anesthesia, will not be included. If you qualify and decide to join, you can expect to experience VR during your surgery, which may help you relax and feel more at ease. This study is currently recruiting participants, and it aims to find a better way to improve the surgical experience for older adults.

Eligibility criteria
- Inclusion Criteria:
- • Patients age 60 or older undergoing primary total knee replacement surgery at BIDMC and receiving spinal/regional anesthesia
- • English or Spanish speaking patients
- Exclusion Criteria:
- • Complex or revision surgeries
- • Patients scheduled for "same day" TKA
- • Open wounds or active infection of the face or eye area
- • History of seizures or other symptom linked to an epileptic condition
- • Patients who plan to wear hearing aids during the procedure
- • Patients with a pacemaker or other implanted medical device
- • Droplet or airborne precautions (as determined by local infection control policy)
- • Non-English speaking or Non-Spanish speaking
- • Moderate to severe dementia
- Drop-Out Criteria:
- • MoCA \<10 (this test will be administered on Day of Enrollment)
