The TRICS-IV trial is studying how different blood transfusion strategies can affect younger patients who are undergoing heart surgery. Specifically, it looks at patients aged 18 to 65 who are considered at moderate to high risk for complications during surgery. The goal is to find out which transfusion approach works better for these patients.
To participate in this trial, individuals must be planning to have heart surgery that involves a machine called a cardiopulmonary bypass, which helps support breathing and circulation during the operation. Participants will need to give their consent and meet certain health criteria related to their heart risk. Those who are pregnant, refuse blood products, or are having specific types of surgeries, such as a heart transplant, are not eligible. If you join the study, you will be randomly assigned to one of two transfusion strategies and will be closely monitored throughout your surgery and recovery. This research aims to improve care for patients like you and may help shape future practices in heart surgery.

Eligibility criteria
- Inclusion Criteria:
- • 1. ≥18 and ≤65 years of age
- • 2. Planned cardiac surgery using cardiopulmonary bypass
- • 3. Informed consent obtained
- • 4. Preoperative European System for Cardiac Operative Risk Evaluation (EuroSCORE I) of 6 or more
- Exclusion Criteria:
- • 1. Patients who refuse participation
- • 2. Patients who are unable to receive or who refuse blood products
- • 3. Patients who are involved in a preoperative autologous pre-donation program
- • 4. Patients who are having a heart transplant or having surgery solely for an insertion of a ventricular assist device
- • 5. Pregnancy or lactation (a negative pregnancy test must be obtained prior to randomization for women of childbearing potential)
