This clinical trial is focused on understanding how to best manage patients who experience low blood pressure (hypotension) due to traumatic injuries before they reach the hospital. Specifically, it aims to explore the effects of different emergency care techniques—like prehospital blood transfusions and a method called REBOA, which temporarily stops blood flow to reduce bleeding—on patients' recovery and survival rates once they are hospitalized. The study is important because, currently, there isn't enough solid evidence about the best practices for treating these patients in the field.
To participate in this trial, individuals must be over 18 years old and have experienced a serious injury that caused low blood pressure, which is recognized by the emergency medical team upon arrival. Unfortunately, those who are already in cardiac arrest (when the heart stops beating) when the emergency team arrives cannot take part. Participants can expect that their medical information—like the type of injury they sustained, the treatments they received, and their recovery progress—will be carefully recorded and analyzed. This study aims to improve future emergency care for trauma patients and ultimately save lives.

Eligibility criteria
- Inclusion Criteria:
- • Age \> 18 years
- • Witnessed traumatic event managed by HEMS
- • Shock at first evaluation (Systolic blood pressure \< 90 mmHg)
- • Suspect or obvious ongoing haemorrage
- Exclusion Criteria:
- • Patients in cardiac arrest at HEMS arrival in which resuscitation is not started or interrupted by HEMS crew
