This clinical trial is looking at how a special video program can help patients with serious illnesses understand advance care planning (ACP) and hospice care better. Advance care planning is about thinking ahead to make sure that your healthcare wishes are known and respected, especially when facing a life-limiting illness. The researchers want to see if this video can improve knowledge and help patients make more informed choices about their care at home.
To be eligible for this study, you need to be at least 21 years old and have a health condition that gives you about a year or less to live, as determined by a specific health assessment tool. The trial is open to all genders and people who speak English or one of the 25 other languages the video is available in. However, if you have serious vision problems or certain mental health conditions that make it hard to participate in discussions about your care, you wouldn’t be able to join. If you decide to participate, you can expect to watch the video and then discuss what you learned about your care options, which could help guide your future healthcare decisions.

Eligibility criteria
- Inclusion Criteria:
- • Aged 21 or over
- • Have a one-year prognosis or less as determined by Clover Health's validated algorithm
- • Speak either English or one of the other 25 languages in which the video intervention is available
- Exclusion Criteria:
- • Visually impaired beyond 20/200 corrected
- • Acute psychiatric illness or other comorbid condition that impairs cognitive ability to participate in advance care planning discussion as determined by the primary House Call Provider.
