This clinical trial is looking at how using a special type of ultrasound, with remote expert guidance (called tele-supervision), can help doctors diagnose and treat patients who are having trouble breathing, known as acute dyspnoea. The study will involve 350 patients who are experiencing this condition. Half of the patients will receive the ultrasound with the support of a remote expert, while the other half will have the ultrasound done without this support. The goal is to see if the remote guidance improves the diagnosis and treatment of these patients, as well as how it affects the stress levels of the medical staff.
To be eligible for this study, patients must be adults aged 18 or older who are suffering from acute dyspnoea and show at least one sign of respiratory distress, such as a fast breathing rate or low oxygen levels. However, patients will be excluded if they are under 18, if performing the ultrasound could delay urgent treatment, or if they do not want to take part in the study. Participants can expect to receive care that includes the ultrasound, and their experience will contribute to understanding how tele-supervision can improve emergency medical care in patients struggling to breathe.

Eligibility criteria
- Inclusion Criteria:
- Patients will be included, if they suffer from acute dyspnoea and at least one of the two following signs of respiratory failure:
- • Respiratory rate \>20 breaths per minute
- • Peripheral oxygen saturation without oxygen supplementation \<90%
- Exclusion Criteria:
- • under the age of 18
- • if PEU will lead to a delay of live-saving treatment or transportation
- • if patient rejects the PEU, the patient will be excluded
