This clinical trial is studying how weight reduction can help people who are overweight or obese and suffer from chronic low back pain. If you are between 18 and 65 years old, have been experiencing non-specific low back pain for at least three months, and have a body mass index (BMI) of 25 or higher, you may be eligible to participate. The trial will take place online, and you will be randomly assigned to one of two groups for 10 weeks. One group will follow a weight reduction program along with specific education about pain and tailored exercises, while the other group will receive similar education and exercises but without the weight loss component.
During the trial, you'll have access to online educational materials and participate in video calls with a physical therapist to support your progress. The main goal is to see if losing weight can reduce pain intensity and improve your overall health and activity levels. Participants will be kept unaware of which group they’re in, and the research team will also not know, to ensure fair and unbiased results. If you are interested and meet the criteria, this could be a great opportunity to find help for your back pain while also working on your weight.

Eligibility criteria
- Inclusion Criteria:
- • Overweight (BMI ≥ 25 kg/m2) or obese (BMI ≥ 30 kg/m2) with chronic low back pain
- • Non-specific low back pain for at least 3 months' duration
- • Currently seeking care for low back pain
- • Native Dutch speaker
- • Access to internet and computer/tablet/smartphone
- Exclusion Criteria:
- • Leg pain of 7 or higher (on a maximum of 10) on a numeric rating scale
- • Evidence of specific spinal pathology (e.g., hernia, spinal stenosis, spondylolisthesis, infection, spinal fracture or malignancy)
- • Evidence of a severe underlying comorbidity (e.g. diabetes, cardiovascular problems, metabolic diseases)
- • BMI ≥ 40 kg/m2
- • Being pregnant or given birth in the preceding year
- • Currently receiving dietary or exercise interventions
