The GPS Project is a clinical trial designed to improve the care of seniors with major neurocognitive disorders, such as dementia, by focusing on their medication management and overall support. The goal is to help seniors take fewer medications, avoid unnecessary drugs, and make their healthcare experience easier. This is done by fostering better teamwork among healthcare professionals like nurses, pharmacists, and doctors, and by working closely with home care services. The project aims to enhance the quality of life, independence, and satisfaction for both seniors and their families.
To participate in this study, seniors aged 65 and older who are either undergoing cognitive evaluations, have been seen at a memory clinic, or have received a diagnosis of cognitive impairment in the past year may be eligible. Participants should be taking prescription medications and will be involved in knowledge-sharing sessions with healthcare providers. However, seniors receiving palliative care or those who cannot complete questionnaires in French without a caregiver are not eligible. Throughout the trial, participants can expect to receive tailored support aimed at improving their health and well-being.

Eligibility criteria
- Inclusion Criteria:
- • All seniors (65 years of age or older) undergoing cognitive evaluation OR referred to a memory clinic OR having been diagnosed with cognitive impairment within the last year OR with MCND and followed up at home AND,
- • referred to the pharmacist, for the FMGs exposed
- • taking prescription medications
- Exclusion Criteria:
- • Seniors in palliative care OR
- • unable to answer questionnaires in French AND without a caregiver.
