This clinical trial is looking at the use of a special imaging technique called intravascular ultrasound (IVUS) to help doctors perform a challenging heart procedure known as percutaneous coronary intervention (PCI) for patients with chronic total occlusion (CTO) of the coronary arteries. CTO happens when a major artery is completely blocked for more than three months, leading to symptoms like chest pain. The study aims to compare the outcomes of procedures guided by IVUS versus those guided by traditional methods to see if IVUS can improve results, such as reducing symptoms and improving heart function.
To be eligible for this trial, participants must have significant coronary artery blockages that have been identified through imaging tests and are experiencing symptoms despite taking heart medications. However, patients with recent heart attacks, kidney issues, or who are in unstable health are not eligible. Those who participate can expect to receive care for their heart condition while contributing to valuable research that may help improve treatment techniques for others in the future.

Eligibility criteria
- Inclusion Criteria:
- • - All coronary CTO patients in whom coronary anatomy is defined by coronary CT and/or Coronary angiography provided that:- CTO defined as heavy atherosclerotic occlusion with TIMI (Thrombolysis in Myocardial Infarction) 0 flow other than via collaterals for \>3 months and they are symptomatic despite optimal medical therapy and/or positive high risk stress modality.
- Exclusion Criteria:
- • Acute coronary syndrome within 3 months.
- • Patients with renal insufficiency (eGFR \< 60 ml/kg/m2, serum creatinine ≥ 2.5 mg/dL, or on regular dialysis).
- • Patients with expected post CTO-PCI procedure SYNTAX \>10.
- • Hemodynamically unstable patients.
