The Portsmouth CORE-D trial is studying how to improve care for adult hospital patients who may be at risk of health problems during their stay. Researchers are creating a large database that collects general, non-personal information about patients—like age, health conditions, and test results—along with details about the care they receive and the outcomes of that care. This information will help develop better ways to identify patients who might need extra support and to test new treatments to enhance patient care and safety.

To be eligible for this study, you need to be an adult receiving care at the hospital. However, if you’ve opted out of research with the NHS or have asked the hospital to keep your records private, you won’t be able to participate. If you join, your data will be used to help researchers understand how to better care for patients in the future. This study is currently recruiting participants of all genders and ages 18 and older, and your involvement could contribute to significant improvements in hospital care for everyone.

• Inclusion Criteria:
• • Adult presenting at the acute hospital or receiving services from the hospital
• Exclusion Criteria:
• • Adults who have registered an opt out of research with National Health Service (NHS) Digital
• • Adults who have contacted the hospital to ask that their records are not to be included in this study
