Transplant Renal Artery Stenosis: Observation Versus Stenting

This clinical trial is studying a condition called transplant renal artery stenosis (TRAS), which is when the main blood vessel supplying blood to a kidney transplant becomes narrowed. This can lead to problems like high blood pressure or kidney dysfunction. The researchers want to find out whether treating this condition with a procedure called IADSA (which might include placing a stent to open the narrowing) is better than simply observing patients with mild to moderate symptoms over time. They are looking for 36 participants who have been diagnosed with TRAS and are at least 18 years old.
If you join the study, you will be randomly placed into one of two groups. One group will get the IADSA procedure, while the other group will be closely watched without immediate treatment. After a year, the researchers will compare how well each group’s kidney function is doing. It's important to note that people with severe symptoms, certain health conditions, or who are on dialysis won’t be eligible for this trial. This research aims to improve understanding and treatment options for patients with TRAS.

Eligibility criteria
- Inclusion Criteria:
- • - 1. Renal transplant recipient with a diagnosis of TRAS by radiological imaging, and an MDT decision to proceed with diagnostic IADSA.
- • 2. Aged 18 years and over 3. Able to give informed consent
- Exclusion Criteria:
- • - 1. Estimated GFR \<10mls/min/1.73m2 or dialysis dependence 2. Contraindication to angiography (e.g. allergy to radiological contrast) 3. Patients with clinical features of severe TRAS (eg. resistant hypertension, pulmonary oedema and/or rapidly deteriorating function).
- • 4. Any condition or co-morbidity which in the investigator's opinion would make the patient ineligible for the trial or unlikely to adhere to trial procedures.
