This clinical trial is exploring the effectiveness of three different treatment options for veterans struggling with alcoholism and substance use disorders. The goal is to find out which treatment helps improve recovery the most. The three options being studied are: a group program called the Combined Recovery Program (CRP), which consists of six sessions while patients are in the hospital; a Home Telehealth program called Stable and Able (S&A), which offers support for up to three months after discharge; and the usual treatment currently provided in the hospital, known as Treatment-as-Usual (TAU).
Veterans aged 65 to 74 who have been diagnosed with a substance use disorder and have used substances in the month before their hospital admission may be eligible to participate. Those interested will start with the CRP or TAU during their hospital stay and may continue with the S&A program once they leave. Participants will be asked to complete follow-up assessments one and three months after their treatment. It's important to note that individuals with certain conditions, such as severe hearing or vision impairments or acute psychosis, will not be able to join the study. This trial aims to help improve support for veterans as they work towards recovery.

Eligibility criteria
- Inclusion Criteria:
- • Meeting DSM-V criteria for current SUD diagnosis of alcohol and/or illicit drug use disorder
- • Use of substances in past 30 days prior to date of index inpatient admission
- • Able to comprehend English
- • Able to provide informed consent
- • Functioning at an intellectual level sufficient to allow accurate completion of all assessments
- • Willing to commit to 6 group inpatient therapy sessions, telehealth S\&A, as well as baseline, and 1-and 3-month follow-up assessments
- Exclusion Criteria:
- • Auditory or visual impairment that would interfere with study procedures
- • Inability to speak or understand English
- • Acutely psychotic patients
