This clinical trial is studying a new approach to kidney transplantation that combines a kidney transplant with a bone marrow transplant from a sibling. The goal is to help the body accept the new kidney without needing long-term medication to suppress the immune system, which can have side effects. In this trial, the donor's bone marrow cells are collected beforehand, and after the kidney transplant, the recipient will receive treatments to prepare their body to accept the new cells. If the treatment is successful and the body starts to recognize the new cells, the need for immune-suppressing medication might be reduced or stopped completely.
To be eligible for this trial, participants must be over 18 years old and have a matched sibling who can be a donor. They should not have any serious health issues that would prevent them from receiving specific treatments used in the study, such as thymoglobuline, which helps to prepare the immune system. Pregnant or breastfeeding women, as well as those with certain infections or cancers, cannot participate. Participants can expect to undergo a series of treatments and monitoring after their kidney transplant to see if the new approach works for them.

- Inclusion Criteria:
- • Age older than 18 years
- • Matched siblings
- • No contra-indication to thymoglobuline or total lymphoid irradiation
- Exclusion Criteria:
- • Pregnant women or breast feeding
- • Infection with HIV, HBV or HCV
- • Previous or presnt malignancy
