This clinical trial is looking at a way to help doctors determine the best fluid treatment for children and newborns who are at risk of developing acute kidney injury (AKI) while they are in the hospital. Patients who are admitted to specialized care units due to sepsis or after certain surgeries will be monitored closely. The goal is to use specific tests and assessments to identify those who may become too fluid overloaded and develop severe kidney problems, which can be serious.
To be eligible for this study, patients must be admitted to either the Pediatric Intensive Care Unit (PICU), Cardiac Intensive Care Unit (CICU), or Neonatal Intensive Care Unit (NICU) at a participating hospital. However, those with severe chronic kidney disease, recent kidney transplants, or those who need immediate kidney treatment before even entering the ICU will not be included. If a child participates in this trial, doctors will carefully monitor their condition and adjust treatments as needed to prevent serious complications related to their kidneys. This trial is not yet recruiting participants, but it's an important step toward improving care for vulnerable children.

Eligibility criteria
- Inclusion Criteria:
- • Admitted to the Pediatric Intensive Care Unit (PICU), Cardiac Intensive Care Unit (CICU), or Neonatal Intensive Care Unit (NICU) at participating institution
- Exclusion Criteria:
- • Baseline Chronic Kidney Disease (CKD) Stage IV or V (estimated GFR \<60 mL/min/1.736m2)
- • History of kidney transplant within the previous 90 days
- • Ongoing AKI or Acute Kidney Disease (AKD) at ICU admission requiring renal replacement therapy initiated prior to ICU admission
- • Acute DNR order or clinical team is not committed to escalating medical care
- • Anticipated to require intensive care for less than 48 hours
