This clinical trial is looking at a program called CONTINUUM, which aims to help patients with advanced cancer transition smoothly from the hospital back to their homes. After being discharged, participants will have a video visit with an oncology nurse practitioner within three business days. The study is trying to find out if this support helps patients feel better and manage their care more effectively after leaving the hospital.
To be eligible for this trial, participants need to be at least 18 years old and must have been hospitalized for an unexpected reason related to their advanced cancer, such as breast, thoracic, genitourinary, or gastrointestinal cancer. They should also be receiving ongoing care at Massachusetts General Hospital and have access to the internet or a phone. It’s important to note that this study is not for patients who were admitted for planned reasons, those who are going to hospice care, or those who were discharged to a place other than home. If you or a loved one qualify, this could be a great opportunity to receive extra support during recovery.

Eligibility criteria
- Inclusion Criteria:
- • Age \>= 18 years
- • First unplanned admission to the oncology service at Massachusetts General Hospital (MGH) since diagnosis of advanced cancer
- • Known diagnosis of advanced breast, thoracic, genitourinary, or gastrointestinal cancer
- • Receiving ongoing oncology care at MGH
- • Verbal fluency in English
- • Internet/wifi or telephone access
- • Residing in Massachusetts at the time of hospital discharge
- Exclusion Criteria:
- • Admitted electively
- • Discharged to a location other than home or expired during admission
- • Discharged with hospice services
